Typically, SLCs are held after a sufficient body of student work has been generated to show that the student is struggling. SLCs may not be necessary if the student is meeting performance goals and achieving well in a class, but may be held anyway in order to keep parents and other stakeholders informed.
SLCs are not always about bad news; sometimes they are an opportunity to emphasize achievement and reinforce good behavior through recognition of good work.
